Leave all keys to the vehicle that we may need, do not leave other keys. Used (LKQ) or non-original equipment (after-market) parts will be installed as specified by the repairing insurance company’s estimate. This vehicle is being repaired for YOU as the OWNER. When the vehicle is ready to be released we look to you for payment via cash, personal check, credit card or insurance check. E.L.B. is not responsible for theft of or damage to valuables left in the vehicle. WARNING. Be sure your check is properly endorsed. Lienholders and other parties listed on the check and required for endorsement are YOUR responsibility prior to giving it to us. All personal and business checks require a driver’s license. There is a $35 fee for returned checks. Some insurance companies will only pay authorized supplements to the vehicle owner. You may have to pay this additional cost to us while waiting to be reimbursed by the insurance company if you pick up the vehicle. Vehicles not repaired by us are subject to a $65 per day storage fee. This fee also applies to repaired vehicles left over 72 hours following completion. After 30 days, interest at the rate of 1.6% will be charged to the balance owed. A research company may contact you to collect customer satisfaction information. I hereby authorize the above repair work to be done along with the necessary material and hereby grant you and/or your employees' permission to operate the vehicle herein described on streets, highways or elsewhere for the purpose of testing and/or inspection. An express mechanic’s lien is hereby acknowledged on the above vehicle to ensure the amount of repairs hereto. E.L.B. is not responsible for unavailability of parts or delays in parts shipments nor loss or damage to vehicle or articles left in vehicle in case of fire, theft or any other cause beyond our control. YOU WILL BE GIVEN AN ESTIMATE BEFORE ANY WORK IS PERFORMED, UNLESS OTHERWISE ARRANGED.
